Mukhya Mantri Lok Sevak Arogya Yojana Assam: Honorable Chief Minister of Assam Hemant Biswa Sharma has started the Assam Mukhyamantri Lok Sevak Arogya Yojana on 02 October 2023 to protect the health of the government employees, retired employees and their families of his state.

Under this scheme, health care up to Rs 5 lakh will be provided to the beneficiary and his family members. The benefit of this scheme will be given to government and private hospitals.

This scheme is for all government employees and retired employees of Assam state. All employees and every member of their family will get the benefit of this scheme.

According to this scheme, if someone benefits from other health schemes of the government, he is ineligible to take benefit of this scheme.

Assam Chief Minister Lok Sevak Arogya Yojana Objective:

1. Health Protection: The main objective of this scheme is to provide health protection to government servants i.e. government employees and retired government employees.

2. Health Service: According to this scheme, cashless health services are to be provided to government employees and their families.

3. Family Health Protection: When government employees retire, their health and the health of their families have to be protected.

4. Health Care & benefits: The Government of Assam has to provide health care and health benefits to the pensioner government employees and their families.

Assam Chief Minister Lok Sevak Arogya Yojana Features:

1. Health Service Govt.Employee & his Family: The benefit of this scheme will be available to the families of all government and retired employees of the state.

2. No Cost: this scheme’s benefit will be non of cost from the government.

3. Health Services: According to this scheme, health services will be available in all government and private hospitals of the state.

4. Cashless Medical Treatments: Under this scheme, all medical treatments, surgeries, operations, and medicines will be available cashless i.e. free of cost.

5. Health Service for Family Women: This scheme also includes child health services and women’s maternity services.

6. Mental Health Service: Mental health services are also covered in this scheme.

7. Eye & Dental Care Service: According to this scheme, the beneficiary and his family members can avail of eye and dental health benefits.

Eligibility for Assam Chief Minister Lok Sevak Arogya Yojana:

1. Citizenship: For this scheme, the applicant must be a permanent citizen of Assam state.

2. Government Employee: The applicant must be a government employee or a retired government employee.

3. Not Be Suspended Employee: Employees suspended from government service will not get this benefit.

4. ID Proof: The applicant must have ID proof of government service or retirement document.

Necessary Documents of Assam Chief Minister Lok Sevak Arogya Yojana:

1. Identical proof: applicant’s Aadhar card.

2. Residence Proof: Ration card.

3. Service ID Prof: Government service ID or retirement document for a retired employee.

4. Applicants Photos: Passport size 2 photographs.

5. Residence Certificate: permanent resident certificate.

6. Bank Account: Applicant’s bank account (Aadhaar linked).

7. Cell Phone Number: Mobile number (Aadhaar linked).

The application process for Assam Chief Minister Lok Sevak Arogya Yojana:

There is only an online process to apply for this scheme. Everyone needs to note that this is not an offline process. The process of how to apply online is given below, follow it.

1. Find Website: First, the applicant has to visit the official website of the Assam Government https://mmlsay.assam.gov.in/.

2. Log in Page: The home page will open in front of you. On the right side of this home page, you will find two such options: Registration and Login.

3. If you have already registered, you can log in by entering your ID and password in the Login option.

4. Registration: If you have come to this website for the first time, then you must register by clicking on the registration option.

5 . Log-In: After that, you have to log in by entering your ID and password.

6. Find Application Link: After that click on the Assam Chief Minister Lok Sevak Arogya Yojana (mmlsay) application link.

7. Fill Personal Details: After that, the applicant has to include full name, date of birth, father’s name, residential address, Aadhar card number, mobile number, ration card number, and family member’s information and include their Aadhar card number.

8. Upload Documents: After that, the necessary documents i.e. passport size photo, resident certificate, service ID or retirement document for retired employee, permanent resident certificate have to be uploaded in the form (document memory given in the form).

9. Bank Details: After that bank details have to be included.

10. Declaration: After that, you have to read and tick the declaration given below.

11. Submit: Finally click on the submit button.

12. Acknowledgment: After that, you will receive the receipt number of the application.

13. Approval: After that, you will receive the Chief Minister Lok Sevak Arogya Card in a day or two. This message will also come on your mobile.

14. Download & Print: After, entering your application receipt number, download and print your health card and keep it with you safely.

When you have to go to the hospital for health care, then you have to take it with you and you can get its benefits by showing it at the health care center.
